University of Toronto Medical Certificate

If you are unable to complete homework or if you miss the midterm test due to major illness or other circumstances completely outside of your control, have your doctor complete an official U of T Medical Certificate and submit it to your instructor as soon as possible for special consideration.

IMPORTANT: Notify your instructor as soon as possible if you find yourself in such a situation. It is always easier to resolve situations earlier rather than later.

For worked marked electronically, please submit the online remark request form via e-mail to the instructor.

For worked marked on paper, please submit the paper remark request form (link to appear) along with the marked piece of work to your instructor.
